Changes with Adair County Home Care Services

GREENFIELD – Adair County Memorial Hospital Board of Trustees has announced it will be partnering with HCI Care Services and Visiting Nurse Services of Iowa (HCI/VNS) to provide skilled nursing care, home health aide and homemaker services in Adair County. This change will take place Jan. 1.

After this change, Adair County Health System (ACHS) will no longer be providing home skilled nursing care, home health aide or homemaker services. These services will be provided by HCI/VNS.

The decision to contract Home Care services was made in effort to allow ACHS to focus on community outreach and public health. The Public Health services will remain a department of Adair County Memorial Hospital.

Selection of a provider is the patient’s choice. Those wanting to receive services from the contracted company, HCI/VNS will not need to transfer records. The current patients of Adair County Home Care will automatically be referred to HCI/VNS, unless a request is received to transfer to another agency by Dec. 18.

A patient who desires records to be transferred to a different agency will need to sign a release of information form. These release forms are available at Adair County Memorial Hospital, 609 SE Kent St., in Greenfield or by calling 641-743-7228 to request a form be mailed.
